The Moms' House and YUP! (Youth
Unlimited Partnership) programs are helping many teen
mothers and high-risk youth get past their rough starts.
Our goals are happy & healthy babies, nurturing
parents, AND productive, happy young people, with [at
least] their high school educations on track and a
priority.

tests done on yourself and your child within the
past 30 days.

18 years of age, numbers 9, 10 & 11 must be

Parent must complete parental permission form
and have it notarized.

The Moms' House must see and obtain a copy of
your birth certificate.

11.
The Moms' House must see and obtain a copy of
your parent's drivers license.

12.Upon
approval by Director/staff, applicant's info
will be taken to weekly house meeting for
discussion with residents.

A meeting with applicant and current residents
will then be set up.

14.Final
decision for approval will be made at the next
residents' House Meeting.

2. Residents' Commitments:
An
addendum to the move-in contract

Attend school regularly. |

Cook dinner for other residents once a week. |

in a timely manner. |
| 4. Keep personal area/bedroom
neat, clean and presentable. |

| 6. Set and keep an accurate
budget. |
| 7. Be respectful to your
housemates. |
| 8. Respect the confidentiality
and privacy of your housemates. |
| 9. Use structured conflict
resolution methods when necessary. |
| 10. Abide by house curfew. |
| 11. Attend weekly house
meetings. |

Support
the accomplishments of your housemates,
i.e., award ceremonies, recitals, etc., and
the parties planned to celebrate these
accomplishments. This may include assisting
in planning the parties. (House meeting

Attend
all scheduled presentations, i.e., parenting
classes, nutritionist, DSHS/CPS, support
enforcement, pediatrician, Planned
Parenthood, career advisors, income
management, Independent Living Skills, etc. |

Attend
scheduled cooking presentations. |

- We will
invite community, resident's
family, teachers, etc., to come
cook dinner approximately once
every two weeks to help residents
with ideas for and opportunities to
learn how to cook economical and
healthy meals.

Assist
in planning, and participate in quarterly
Open House. |

- We will
invite parents, teachers and
community to coffee, treats and
different discussion topics. (House
meeting agenda item)

Assist
in planning, and participate in fund raising
activities. (House meeting agenda item) |

Last,
but certainly not least, we know that you
want to give your child the best care you
possibly can. This will include keeping
current with your child's well-baby
appointments, shots, and any other services
you have been referred to by your child's
primary health care provider. |

Non-compliance
will, at the very least, warrant a conference with the
House Advisor and/or the Director and that a
Non-Compliance Conference Agreement is completed.

(School
night quiet time is 8:00 p.m.) Quiet Time
means that you act as if your roommates are
sleeping and house noise is kept at a minimum. |

2. Overnight
stays away from The Moms' House.
After
a three week settling in period.
Not
more than three times per week. If resident is
gone that often on a regular basis, need to
look at helping them to move on.
Up
to a week's planned vacation away from house
after three months. Must obtain advance
approval.
3. Dirty
clothes are to be kept in an acceptable container and
washed on a regular basis.
4. No perishable food items are to be kept in bedrooms.
5. Residents must submit to a search of all personal
areas at any time.
6.The resident agrees to give a written 30 day notice to
The Moms' House of intent to move out.
7. No smoking inside the house.
8. No incoming phone calls after 10:00 p.m.

Non-compliance

3. Guests must be respectful to other residents and
neighbors.
4. No loud music from cars, honking, etc.
5. Visitors may visit in bedrooms with an open door
policy.
6. No visiting 5:00 p.m. - 7:00 p.m. (dinner preparation
& eating time)
7. Each day, each visitor must pick either time frame A.
or B. to visit, not both.

B. 7:00 p.m. - 10:00 p.m.
Friday
A. 3:00 p.m. - 5:00 p.m.
B. 7:00 p.m. - 11:00 p.m.

B. 7:00 p.m. - 11:00 p.m.
Sunday
A. 10:00 a.m. - 5:00 p.m.
B. 7:00 p.m. - 10:00 p.m.

5. ZERO TOLERANCE
Applies to
residents and their guests.
Guests' behavior will be viewed as the behavior of the
resident.
1. Can never
hold yourself out as a GANG member.
No
wearing colors (flagging), tagging, hitting
people up, NOTHING!!
2. No Drugs
or alcohol on site.
Bringing
or having drugs or alcohol on site, or being
under the influence of drugs or alcohol is
grounds for immediate dismissal and will be
dealt with in accordance with the law. Resident
may be asked to submit to random U.A.'s
3. No
dangerous weapons (as described by law).
Bringing
or having a dangerous weapon on site is grounds
for immediate dismissal and will be dealt in
accordance with the law.
4. Physical
violence is grounds for immediate dismissal and will be
dealt with in accordance
with the law.
5.Theft is
grounds for immediate dismissal and will be dealt with
in accordance with the law.
6.Neglect,
abuse and exploitation of any minor will be dealt with
in accordance with the law.
